*Position Overview:*
We are currently seeking a detail-oriented and results-driven Registered Nurse (RN) to join our team at Galloway Senior Living on a PRN (as needed) basis.
The PRN RN will be responsible for providing oversight on Individual Service Plans (ISPs).
This will involve initial assessments and any subsequent assessments related to changes in residents' conditions, rather than on an annual basis.
This position requires a commitment of 4-8 hours per month to complete visual assessments of residents and co-sign assessments as necessary.
*Responsibilities:*
*Assessment and Care Oversight:*
* Review initial assessments of new residents to evaluate their health status and care needs.
* Review assessments as needed in response to changes in residents' conditions, collaborating with the caregiving team to adjust care plans accordingly.
* Sign assessments conducted by the wellness staff, ensuring accuracy and completeness.
*Communication and Collaboration:*
* Communicate effectively with the wellness team regarding assessment findings and care plans.
* Collaborate with caregivers, CNAs, and other healthcare professionals to provide comprehensive care and support for residents.
* Participate in meetings to discuss resident care plans, changes in condition, and any necessary adjustments.
*Compliance and Documentation:*
* Ensure compliance with all regulatory requirements related to resident assessments and care documentation.
* Maintain accurate and confidential records of assessments and care plans.
* Adhere to ethical standards and best practices in nursing care.
*Qualifications:*
* Valid Registered Nurse (RN) license in the state of IL.
* Minimum of 2 years of experience in nursing, preferably in a senior living or long-term care setting.
* Strong assessment skills with the ability to recognize changes in residents' conditions.
* Excellent communication skills.
* Ability to work independently and collaboratively as part of a multidisciplinary team.
