Department: Case Management
Schedule: Full Time, Day Shift
Hospital: All Saints Spring St Campus
Location: Racine, WI
What You Will Do:
Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.
Create plan for care across the continuum, integrating patient/family preferences and values.
Serve as a resource to assess, evaluate, recommend and treat legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs.
Advocate for resources and removal of barriers.
Maintain ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.
Act as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care.
What You Will Need:
Licensure / Certification / Registration:
Registered Certified Registered Nurse credentialed from the Wisconsin Board of Nursing preferred.
Rehab Therapist specializing in Occupational Therapy (OTR) credentialed from the Wisconsin Medical Examining Board preferred.
Social Worker credentialed from the state of Wisconsin.
Licensed Clinical Social Worker (LCSW) preferred.
Rehab Therapy Assistant specializing in Occupational Therapy (COTA) credentialed from the Wisconsin Medical Examining Board preferred.
Education:
Associate's Degree in related field required.
Bachelor's Degree preferred.
Work Experience:
Minimum of three (3) years clinical experience required.
Experience in an acute care psychiatry environment and/or equivalent knowledge of disease process is required.
