Nurse Practitioner

Reports to a Director for administrative supervision and the Practice Site Physician or designee for clinical supervision.
Responsible for the coordination and delivery of primary care, as specified by the appropriate certifying agency, to patients of specific area employers.
Provides clinical and administrative assistance to programs, etc.
Works collaboratively to meet clinic objectives for patients and clients.

A valid and current RN license in the state of Indiana is required.
The successful completion of a nurse practitioner program with applicable national certification is required.
Two years of clinical nursing experience in practice area and successful completion of a Nurse Practitioner six-month internship for new graduates or comparable experience is required.
Master's of Science Degree in Nursing is preferred.
Attainment of prescriptive authority or successful completion of applicable course work and pass state board exam to obtain prescriptive authority within six months of hire date.
*Bilingual speaking and/or writing skills preferred.

* Requires a high level of analytical skills necessary to assess the patient and participate in follow-up care planning.

* Demonstrates high level of communication skills, both oral and written, necessary for effective communication with patient/family, physicians, and referral sources.
*Coordinates and delivers primary health care and wellness care by:* * Working within the approved scope of practice as designated for this type of professional.
* Obtaining complete medical history and physical data on patients.
* Interpreting and integrating data to determine appropriate diagnostic and therapeutic procedures as needed.
* Performing a complete physical exam and recording findings.
Managing acute and chronic medical needs of patients.
* Ordering appropriate laboratory and diagnostic procedures.
* Synthesizing data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.
* Administering, dispensing, and prescribing medications and injections.
* Interviewing and advising patients regarding health and illness prevention.
Recommending community resources to meet patient and family needs.
* Instructing patient and family regarding medications and treatment instructions.
* Maintaining and reviewing patient records, charts, and other pertinent information.
Posting tests and examination results.
* Triaging patient telephone calls and providing consultation.
* Providing monitoring and continuity of care as appropriate between visits.
* Collaborating with a Medical Physician in accordance with professional guidelines.

*Performs other functions to maintain personal competence and contribute to the overall effectiveness of the clinic by:* * Developing and maintaining affiliation with professional and community organizations.
* Assisting in development and compliance of clinical protocols, policies and procedures.
